ABSTRACT

Objective:To construct a prediction model for difficult tracheal intubation in the patients with obstructive sleep apnea-hypopnea syndrome (OSAHS).Methods:A total of 324 patients with OSAHS undergoing surgery with general anesthesia, admitted to our hospital from June 2019 to June 2021, were included in model group, and 175 patients with OSAHS undergoing surgery with general anesthesia, admitted from July 2021 to July 2022, were selected and served as validation group. The patients in model group were divided into occurrence group and non-occurrence group according to whether difficult tracheal intubation occurred. Logistic regression was used to construct the prediction model, and R4.2.1 software was used to draw the risk nomogram and calibration curve. The predictive accuracy of the model was evaluated by the area under the receiver operating characteristic curve. Results:Body mass index (BMI), sagittal minimum cross-sectional area, horizontal minimum cross-sectional area and mandibular distance were risk factors for difficult tracheal intubation in OSAHS patients ( P<0.05). A prediction model was developed using the above factors: Logit P=33.726+ 1.411×BMI score-0.014×sagittal airway minimum cross-sectional area-0.013×airway horizontal minimum cross-sectional area-0.312× mandibular distance. The area under the receiver operating characteristic curve was 0.846, Youden index 0.585, sensitivity 0.831, specificity 0.755, and the accuracy 0.889 (Hosmer-Lemeshow test χ2=9.24, P=0.322) in model group. The area under the external validation curve was 0.802, Youden index 0.545, sensitivity 0.636, specificity 0.908, and the accuracy 0.893 (Hosmer-Lemeshow test χ2=10.24, P=0.287) in validation group. Conclusions:The prediction model based on BMI, sagittal minimum cross-sectional area of airway, horizontal minimum cross-sectional area of airway and mandibular distance has a high value in predicting the risk of difficult tracheal intubation in patients with OSAHS.

ABSTRACT

BACKGROUND: Mechanical stimulation plays a necessary regulatory role in developing and repairing many organs and tissues in the human body. Except for biochemical factors, mechanical factors are considered as key regulatory factors that affect the behavior and function of dental pulp stem cells. OBJECTIVE: To review the role and effect of cellular mechanical stimulation on the biological behavior of dental pulp stem cells. METHODS: PubMed, Embase, Medline and CNKI databases were searched for relevant literatures using the keywords of “human dental pulp stem cells (hDPSCs), mechanical strain, mechanical stretch, mechanical tension, shear stress, cell proliferation, osteogenesis differentiation” in English and Chinese, respectively. Fifty-six articles were finally eligible for review, which were closely related to the proliferation and differentiation of dental pulp stem cells under cellular mechanical stimulation. RESULTS AND CONCLUSION: Cellular mechanical stimulation is an important biological factor affecting cell proliferation, differentiation, apoptosis and protein expression. Dental pulp stem cells are mesenchymal stem cells derived from the dental pulp tissue, and their biological behaviors are also affected by cellular mechanical stimulation. Cellular mechanical stimulation is involved in the proliferation, odontogenesis/osteogenesis of dental pulp stem cells. When the dentin is subjected to fluid flow forces, mechanoreceptors are activated to regulate and maintain the integrity of tooth structure. Signal pathways that mediate the biological behavior of dental pulp stem cells include MAPK, Wnt, Akt, BMP-7, and Nrf2/HO-1, which are involved in promoting and inhibiting the proliferation and odontogenesis/osteogenesis of dental pulp stem cells to varying degrees.

ABSTRACT

This study was aimed to explore the effect of Catalpol on the cerebral infarction size in acute phase, water content and inflammatory reaction of early recovery after permanent middle cerebral artery occlusion (pM-CAO). Adult male Sprague-dawley rats were subjected to chemical method to establish MCAT model. The detec-tion was made on neurobehavioral symptoms, cerebral infarction volume and water content at 24 h after surgery. The content of intedeukin-6 (IL-6), intedeukin-10 (IL-10) and nuclear factor kappa Bp65 (NF-κBp65) were de-tected after pMCAO with enzyme-linked immunosorbent assay (ELISA). The results showed that 24 h after MCAT, Catalpol 15-60 mg·kg-1 can significantly improve the neurobehavioral symptoms (P < 0.01, or P <0.001). The Catalpol 15 mg·kg-1 can significantly reduce cerebral infarction volume (P < 0.05). The Catalpol 30-60 mg·kg-1 can significantly reduce water content (P < 0.05). Catalpol 30-60 mg·kg-1 can significantly improve neurobehav-ioral symptoms from the 7th day after pMCAO. On the 14th after pMCAO, the content of IL-10 and NF-κBp65 of ischemia brain had no difference compared with sham-operated group. The IL-6 level of ischemia brain was obvi-ously reduced than the sham-operated group(P < 0.05). The intragastric administration of Catalpol 15 mg·kg-1 for 14 days can reduce the content of NF-κBp65 in the ischemia brain of model rats (P < 0.05). It was concluded that Catalpol can improve neurobehavioral symptoms of acute and subacute phase after cerebral ischemia, reduce infarcts and water content. These effects may not be related with its inhibition of inflammatory derived from cere-bral ischemia.
